A Note from The Virtual Wall

On 15 Sep 1971 an OH-6A (tail number 68-17175) from A Troop, 3/17 Cavalry was conducting a visual recon of areas where a troop insertion was to be conducted. As the door gunner, SP4 Charles Egbert Jaquins, prepared to mark a bunker with a white phosphorous grenade, the grenade prematurely exploded. The heavy smoke filling the aircraft caused the pilot to lose control and the aircraft crashed. Although injured, the pilot and observer were able to exit the burning aircraft and extract SP4 Jaquins, but a week later Jaquins died from his injuries.
